Currently JT and I have discussed that in the future we want to
- control a limited set of NotEye features directly from ADOM (e.g. the actual game configuration settings like full screen vs. window, ...) and turn the rest invisible for the user as the multitude of options probably is overwhelming for the target group at Steam.
- provide game events that advise NotEye to change a setting.
- implement a versioned setting file format for ADOM.

I had been thinking that all visible options would just be in an "options" item on the main menu -- whether they be noteye, or the adom.cfg, laid out by category. (Possibly with sub-menu's for some of it.) For example (a random selection .. you might prefer some of them to be invisible):

Sound
Music Volume
Sound Volume
Graphics
Inventory Graphics on/off
Show Animation
Show Blood
Tile Size
Gameplay
Automatically Open Doors
Automatically Pick-up These Items (when auto-pickup is turned on):
amulets, arrows, scrolls, gold, books, rings
No Book Spellcasting
Mouse control
Auto-Centering Speed ---------|- (that's a slider) :)
Manual scrolling region -----|------
Manual scrolling speed -|----------
Scroll-Wheel on/off
Information
Coloured Messages
Show Experience
Show Healthbars
Misc
Exit ADOM after player death
Exit ADOM after saving the game

The real challenge, I guess, is what to make invisible. :) ..and, of course, the invisible ones can still be changed in the relevant cfg file.
(I'm just throwing around ideas, of course. ..feel free to use all of it, or none of it.) :)

IMO many if not all of the options currently available in adom.cfg are power user options and will be rarely if ever used by more casual gamers we currently try to get into the game. Thus I personally think that the initial version of a game settings menu only needs the most important things (like e.g. full screen versus window, audio levels for music and FX).

I currently also think that we will have to kinds of settings:
1. The more technical ones mentioned above
2. Game customization settings (corruption, hunger, saving, ...)

Actually, this pleases me a lot. I'm happy to have most of the config options hidden to the average player. ..I think the game will be better for it.

(I would like to see a nice menu for the auto-pickup option, but that feels more like an in-game thing, rather than options material.) :)
