Mighty blow... on a door
A monster like a troll brute can hurt the player with mighty blows delivered on a door.
"The troll brute opens a door. The troll brute delivers a mighty blow! The troll brute bashes at you with a large great club moderately smashing you."

Funny... since the forementioned troll was something like 20 square away...
