They don't. The server uses a hacked version of the game to let you do that.

Does it actually hack the game or does it merely run the game multiple times or set the system time temporarily to a specific value?
If ADOM ever gets updated again this is one of the features I really wish would get added.
You can easily roll characters over and over till you get a sign you like (I do, sometimes) so it's no major advantage... it just saves on annoyance.
TB wasn't in favour of it, for reasons never made entirely clear, but said that if he ever released 'ADOM Deluxe' (a more commercialised ADOM, basically) he'd include it.
Personally, I feel that the fact that the server allows you to pick your own starsign is one of its best features.

I feel that unless you're trying to do something special (speedruns), you shouldn't pick your starsign. After all, the best two starsigns for most classes (mindcrafters might prefer tree) are candle and Raven. Why would you choose any other?
Those might be the best, but then drakeling is arguably the best race and wizard arguably the best class so why pick anything but those?
I'm rather partial to dragon or sword for melee types, for example. Candle and raven may be the 'best' but it doesn't mean the others are not useful and fun for a change.
Anyway you already can (and I imagine many if not most people do) choose your star sign by simply rerolling over and over. It's just tedious.

Sure some are better than others but in 99% of cases does it really matter.
Well, Candle starsign actually makes very significant difference on gameplay for some classes, since it can completely replace the healing skill (and then some), allowing classes lacking both skills to take the druid quest. For the remaining classes, the effect of starsign doesn't matter quite so much, but can make the game much, much, easier. It's like talents: there is no reason that you can't finish the game entirely without talents (as in ADOM 1.0.0), but the benefits of taking a class-appropriate set of talents will give greatly increase PC survival early on and some substantial longterm benefits as well.
For players who have trouble surviving early on, rolling the right starsign is often worth the trouble.
Hoping to win with every class, doomed. Archer, Barbarian, Bard, Beastfighter, Druid, Elementalist, Farmer, Fighter, Monk, and ULE Priest down.
