    Find adamantium halberd of penetration in the SMC only to be killed by double-level summoned blink dogs while in sight of the downstairs... yeah. That one hurt - that was many many years ago but I still remember it vividly.

    However, my first win came when I found an eternium two-hander in the SMC and I didn't die on UD:1

    Generate a dark room together with river on an entrance to level.

    This was my most painful death.
    I'm going to win with Dark Elves of all classes.
    Mindcrafter, Necromancer, Beastfighter, ULE Archer, UNE Barbarian done.

    Always make ID blink dogs summon friends in deadly chilling rooms.

    Generate water traps every second tile when you're carrying lots of spellbooks.

    Make doppelgangers always destroy -conf equipment with the first hit.

    Generate mimics adjacent to shop entrance in dwarftown EVERY time...

    ...and if the PC survives, make dwarven mystic ask to kill a mimic to prove yourself.

    Always put as many dwarven children, goodwifes and farmers in local shops so they can block your every step.

    Spawn deaf/unlife door braking monsters in AF so they can free extremely experienced lvl 110 banshee that will be exactly next to the spot the PC teleports to in order to collect loot.

    Always spawn more white worms on the other side of the level, just when the PC has locked the side corridor with a few jellies inside, while pacifying the big room.

    Increase spellbook drop rate in ID just when PC is strained! so that you get excited like a little kid seeing so many books and not seeing HP dwindling with each descend until you break your neck.

    Generate monsters on your path when returning with Yrrigs so that you have to go another way and step on a fire trap.

    EDIT: Every blink dog would generate a corpse no matter what, but only after you already ate one.

    And my favorite:

    Always generate Serpent's Bite and Skullcrusher in two subsequent UD vaults for assassin players, so they can be tricked into overconfidence while dual-wielding them on PC lvl10 against Hotzy.
