I don't know if this was covered in the 'exploding spellbook': "was drained of his life-force by strong magic" (death by reading a spellbook)
"was transformed into a shadow" (death by a Slow Shadow?)
Updated to now include the book-drain death (should have remembered that when the exploding book came up). "Transformed" has already been included, though I haven't listed all the enemies that can do so since there are many (even living walls, oddly enough).
Just found another one:
"Broke his leg when kicking a tree."
I was curious about what would happen, so I tried it (it was a throw away character anyway).
Bringing this back up - I've got a new one.
Outlaw lead, the orcish paladin, was killed by vile kobold shaman magic.
You can also die by wearing an amulet of order while chaotic or vice versa. If the damage done by doing so is greater then your regeneration rate it will eventually kill you if you don't take it off.
"You feel a burning pain around your neck. You die."
That's already there - #39.

Transformed into a true servant of Chaos. (I believe it happens if you enter the gate while below level 50)
Extinguished by an exploding fluff ball.
Died of exhaustion on a bad day. (Start as a spellcaster with Toughness 1 on Monday, and see if RNG hates you that much)
