Here follows my notes for most of the game. Next post will contain recollections as I stopped taking notes after killing casino shopkeep. It is probably more interesting :) Full flg is at http://ancardia.us.to/adom_users/sirtheta/rosa.flg

SMC - First stop. Found blanket in > room.
UD1 - Farmed unstable morgia/moss/pep/stoma patch, eventually got it into following form (lost moss, gained alraunia)

UD2 - Cleared gnoll vault; gained poison resist from random giant spider spawn. Decided to take up axes, also picked up adamantium shield.
UD4 - Found N altar and changed alignment. Obtained TP control from BDs.
HMV - Leggot is dwarf, so he will die...eventually.
Entered UD w/ St: 16, Wi: 14, To:18, left with St: 18, Wi: 20, To: 23.

On to CoC.

D4 - Cleared minor undead vault.
D6 - Big Room. Stablized some more herb patches even though I didn't really need them.
D9 - Jelly vault. Skipped.
D10 - DT. ID inventory and find that soft boots from minor undead vault are 7LBs. Bless. First quest is ratling master thief.
D12 - Two staircases, so this is as far as I go at this point.
D5 - Surfaced to Arena since I'd bypassed getting food earlier. Did first five fights since had no means of dealing with cats.
D12 - Decide there is only 50/50 chance, so why not try to get to later levels to level and find ratling master thief? Of course, choose wrong stairs. Oh well, I have TH, wand of fireballs will come to me.
DH1 - Very nasty ghost tension room. As expected, my weapon is not up to par, and I get aged 37 years fighting one ghost. Animated forest it is.
D6 - Decided to surface and do Griffyard, hopefully giving me enough exp to do Pyramid. Had the worst luck trying to cross big room - 3 werewolfs, 2 wererats, necromancer..eesh. Made level 12 though, guaranteeing that clearing Griffyard would open up Pyramid. Scored bone bracers, giving nice boost to HP - 170 now.
D1 - Realized I forgot to ask about portal.

GY - Clearing top of GY leaves 12k exp to 13, easily achievable on lower level. Nonnak comes out from hiding while I've retrieved my stuff; I hit him until he panics and the rest is easy when he bumbles over a light trap. Griff is not too difficult, so I take him in melee to conserve water.

PL - Despite my weapon being just a blessed mithril battle axe (+0, 1d6+5), pyramid is a joke; I take more damage from traps than top level (used darkness to slay annoying ice vortex). Grab pickaxe and head back to GY to get sword for saccing.

UCBT - Had wand of cold so decided to do RotHK. Cavernous levels were dicier than I remember - probably due to mediocre damage output. Skeletal king was quite tough, but wasn't confused once and just kept eating pepper petal and wailing on him 'til he died, which he did. Unbelievable blink dog infestation on multiple cavernous levels meant I barely escaped intact. Note to self: find much better weapon.

D14 - Scythe.
D16 - Khelly, exit back to DT for now while I contemplate whether I want to save him
D5 - Now that I have wand of trap creation, decide to do fights 6-19. As luck would have it, encounter kitties both times. Disposed of with acid traps so I don't accidentally bump into them on the level.
D6 - harvest more herbs for sickness scumming and hopefully enough morgia to last me through any bad corruptions. also get a little more stoma+ceptia for starvation scum in ogre caves, should I ever get there. find +9, +7 medium crystal shield and bracers of regen while dealing with annoying summoners. also unID'd fireproof blanket, so I no longer have to leave 7lbs on another level when dealing w/ vortices.
D7 - sense pious aura, find N altar and ID stuff before descending to DT
DT - complete portal quest this time. I now have 44 blessed, 16 uncursed and 19 cursed stoma and lots of ceptia for starvation training. I probably have enough resources to crowned. I need another level before doing ogre caves (and haven't done first quest anyway) so I can pump food pres to at least 90. Bracers of regen come in handy when using sickness, but mostly I wear To bracers for extra HP and +2PV.
D13 - left my tower adamantium shield back at DT, so equip adamantium medium shield i'd found. turns out to be cursed, so temporarily lose 5 PV (31) when wielding weapon. animated forest has become tedious with all the trees now. start hacking through hostile trees out of boredom.
D16 - Khelly dies quickly
D17 - another beehive. a couple more and I'm in serious trouble. more fucking blink dogs (just can't escape from them this game).
D18 - coc graveyard. + ants. I get to their basecamp before they've burrowed through to main graveyard. munching takes me back to C but boost To to 30. could have waited for increase in max to easily get it to 31, but wight corpses are easy munching and didn't want to wait. 30 is enough for now anyway. dig in rather than going around. vampire gets me to L16, I take food pres to 100. 3 vampire corpses manage to boost me to To 32
D20 - water temple. snake comes out early and have trouble dealing with him + water breath + many other creatures to fight. eventually get sandwiched between snake and troll berserker and get hit for first corruption - unholy aura. this is problem as I have much left to do in DT. Two pots of blindness and snake is down, but I have no holy water.

DH2 - quickling king is extraordinarily hard to deal with. after he blind rages, I have to use only scroll of teleport to get back to stairs to make it out alive. animated forest it is.
D13 - unholy aura makes this much more manageable
D8 - manage to do many things in DT without making thrundarr or waldy fear me. got crowned (nature's friend + lightning immunity). semi-good since I plan to go through BD caves. luckily, even w/o dipping, snake made it this far.

Terinyo - Turn in snake for PoCC, being careful to stay away from Munxip. Will keep unholy aura for now. Also get Keethrax quest to obtain another PoCC
DD7 - Keethrax took just two hits, about as expected. Unholy aura managed to let me avoid slaying any cats, though I did accidentally run into one and severely wound it.
Terinyo - Turn in black druid for PoCC. Decide not to complete quest at this time.

VDDL - Realizing I have wand of item detection, I go to grab TP wand so I can upgrade from blessed mithril battle axe (+1, 1d6+7). Use last two potions of holy water to bless wand and 2 scrolls of recharging.
NDC - It takes a while, but I take out golem guarding stairs w/o losing HP except to fire breath. If DFG permits, will clear other golems.
DFG - Good stuff includes 3 quicksilver quarterstaves, murderous eternium spear and broken knife of endurance. Notice my To is now 33 w/o To bracers. Decide to poolsip, see what happens; 9 pools present. +Para +Invis. Almost decided to stop there, but would enjoy teleportitis, so sipped once more - +Doom. No big deal, this is very desirable and Doom is easy to remove since I've just crowned. There are also two pools before DT I can sip if I really want to (probably shouldn't).

D7 - Remove dooming. Sac some of more useless kept weapons to reduce burden status. Decide to dive to Wall of Flames in hope of finding ratling master thief and getting extra healing pots from Thrundarr. Also would like to improve Polearms skill as I intend to murder Wyrm in melee, per usual.
D13 - Murderous spear is very good at murdering trees. Since I have forsaken AMW, I cannot see myself and imagine myself as tiny underline of doom.
D18 - Remember I need to talk to Dwarven Mystic.
D22 - I hear tiny bells chiming. Scroll shop by down stairs. HE shopkeep. Forgot about unholy aura.
D28 - Hello, my pretty! Ratling master thief slain. Now to get rest of quest from Thrundarr w/o angering Waldy.

DT - Encountered two more ratling master thieves on the way back, of course!
OC - Enter: 21 St. Leave: 27 St.
DT - Pot of vis lets me do arena, but I get equipment set. I now have headgear for tower, at least. Trade in golden gladius for Tactics before greater daemon, who dies in two stabs. Accidentally kill dwarf while exiting Thrundarr's house after killing greater daemon. This only makes Waldy hostile, fortunately and confirms my recollections. For now, Waldy lives.

ToEF - despite ACW not staggering so aimlessly as the game would claim, I make it out alive and with only one more corruption.

D31 - Casino. Preserver. Unholy aura upsets shopkeep about 2/3 of the way through and I don't have darkness, so skedaddle.
D34 - Surge of power. Soaker.
D38 - Greater vault of giants. Serpent's Bite. Walk out with 36 St and Daggers at skill 8.
D39 - Threat room of Titans on Air Temple level. Got sweet antennae corruption and gained 4 levels.
D44 - Earth Temple.
D45 - Giant vault. Type with four hidden rooms. Looks like missile weapon and black tome - I skip for now.

D1 - Surface to remove curse from robbing casino and ID stuff since I have no holy water atm.
D32 - Despite having Literacy in the 50s and no Concentration, I manage to get one okay read from Darkness book. Time to kill casino shopkeep.
D31 - Kill casino shoppy. Used darkness and he never even touched me.

This is where I start dual wielding Serpent's bite + phase dagger/sword of sharpness. I did Air Temple next - didn't even know I was meleeing Yulgash when I saw I'd killed him. Earth Temple was very dicey due to stair layout/backing up room. At one point was surrounded and had to resort to serpent's bite + shield. ASB was a pushover though, he only managed to hit me once and he died pretty quick. After fight is over, I quaff Potion of Education to get: Courage! excellent! On the way to Mana Temple, I discover an N altar, which saves me a trip to the surface to bless water and remove cursing from all the shopkeepers I keep pissing off. Mana Temple was unbelievably easy. I forgot to equip AMW at first, but luckily remembered after he tried to confuse me. Didn't wind up mattering as I dodged all his energy bolts. Archmage didn't stat drain me at all - only cursed me, though when I went to remove it at an altar, I was not "relieved," so I don't think I was actually cursed.

Next, surface to kill assassin prince. Backstab with serpent's bite to take to moderately wounded. Forgot to kill intervening assassins first since I was invis, so my route of escape was blocked. Luckily I managed to clear one before he hit me again. He only got a chance to hit me twice, but the second time, including round of poison, I took 300 points of damage (luckily I had equipped Earth orb and had over 600). Assassin prince quickly gives up Sting. Time for BDC which are ridiculously easy - I have shock resist, immunity, and Blue DSM on. On way to ancient blue wyrm (ABW), I kill two giant slugs, one leaves a corpse! ABW barely takes out 200 HP over the course of our battle. Item destruction was almost nonexistent - I'd left my blessed broken rusty knife of endurance on steps, but found 3 more 8s knives while down there and all made it through, including one in missile slot. Gained a PoGA. Killed two greater molochs while there. Obtain Needle from AKW. I start dual wielding sting & needle (previously serpent's bite and sting). Uselessness gives me Hammerhead.

D50 is possibly easiest part of game. I buff up my Black DSM and 7lbs with some scrolls of defense/protection. On very aggressive, I'm at about 71/65 and balors can only touch me on criticals--I melee all of them and get him 3-4 times at most. Fisti is a bit tougher, he regularly hits me, but about 6 turns and he's dead as well. I've gained mana battery, which is annoying, but I don't really need my wands, per se, so I'll keep it for now. Use darkness to clear out chaos wizards/servants/etc in "big room"

At this point, I am at about 68k turns and done with game. I decide to take revenge on shopkeeps though - I've angered 5 so far and killed 4 of them (Waldy is alive). Next shopkeep to die is selling wondrous figurines. Then Leggot, who I promised to kill much earlier. Then Waldy. And finally Munxip just because i could. All angered using unholy aura first.

Lastly, I descend to momma dragon caves. Find her, cast darkness. First turn: slightly. Second turn: moderately. Third turn: crtically. Fourth turn: dead. Didn't even have to true berserk. I steal her stuff just because I can; I don't really need it.

Then, I leave the Chain.

My only real regret during this game is that I had to switch weapons so much. I think if I'd been able to do daggers from the beginning, I would have almost certainly reached Grand Mastery in daggers, or, at the very least, gotten quite close.

I thought the game considered monsters that were killed by traps that you created as killed by you. Or is it only traps that you set off?

It may in fact be only door traps; I do not know for sure whether or not killing someone by setting off a fireball trap gives you xp (mostly because I don't believe I've ever been in a situation where I did it/could do it).
