I was on the catlord level, assassin level 25ish, crowned with executor, everything is groovy. I can feel the win. I just put on some cursed gauntlets but whatever, NBD. Kick a locked door. Fireball Explosion. Dark tension room full of minotaur mages on a non-teleporting level with the stairs on the other side. Couldn't switch out my rings for confusion control and couldn't dip things because i was confused. I final managed to get mostly unconfused (after losing way too many stats by switching in the Water Orb. I survived but my stats were so low that I just couoldn't bring myself to continue.... quit.

Also cat lord level.
My mindcrafter has done everything right, enter D:35, I proceed to seek him out to get my ring
I have preserver and kinslayer.
I find the cat lord indeed, but since I forgot to equip AMW (instead I was wearing nature's companion) I ran into him, stabbing and making him hostile.
Bye bye ring. I built that char specifically to get the ring and paid utmost care all the time to not kill any cats and then this happened. Shift+Q.

Another mindcrafter (I play those a lot).
Dark elven one. Got precrowned with executor, yay. Let's grind some piety for a later crowning proper.
Just after doing the tower, I went to UC:4 where I had a convenient altar in the middle of one of the cavernous levels.
I reached L+ in the tower already so now I need to finish the remaining piety necessary with a few live sacrifices.
Finally, I get crowned with ring of immunity. Happy that I can now dive and unveil the CoC greater vault(s), I move carelessly in berserk mode and 20 turns after crowning I get petrified by gorgon, just behind a corner.

i personally wouldn't quit over stat loss when one can get them to relatively good levels back with herbs, garth and casino gold

yeah the thing described by the op happens to me a lot of times...
i still remember when i started playing adom i reached the water temple for the 1st time and the were 'you hear a distant wind' messages spam on every walk command of mine....today i know what that means but back then i just decided to scout the whole level....found a acid vortice tension room....silly me tried to teleport out after 1st vortice hit me and realised you cant tp on such levels..
next was on air temple...obviously i dropped all destructible items outside and went in (i had NEVER died to yuglash or for that matter asb EVER in around 5 attempts so didnt take him seriously)...neways i open the the main room and he summons.......quickling queens.....RIP barbarian. apparently vanquisher sucks vs quicklings
