Since the game will be such that you'll be able to spend years in it, will the game commemorate the players birthdat? Perhaps the monsters can throw you a birthday party, or the Gods could drop a little cake from the sky.

A little bonus to luck and/or fate smiles on your birthday seems reasonable. Perhaps a heightened chance to get a wish from a pool or something like that.

Until you hit 40. Then your NPC has a midlife crisis and starts riding around on the most expensive war horse he can find.

You know this already exists in ADOM, right? Well, the balors don't throw you a surprise party on D50 and Andor Drakon doesn't bake you a cake, but you get a "Happy Birthday" message and increased luck on that day.

I don't know what game you're playing, but the surprise birthday party on D:50 is the main reason I play ADOM.

"Happy Birthday, x", Fate smiles and "Suddenly a giant cake drop down from the sky/roof" would be nice. If one would like to be really mean the PC could get crushed under the giant cake - being avoided similar to a trap >: )
"Happy Birthday, BAM", "Game over".

The game shouldn't encourage the player to use their birthdays adventuring... That's not the ideal way to spend ones birthday, working? You should go to a local tavern to celebrate. And maybe the bartender would give the PC a free drink or three if he/she's charismatic and pretty enough.

Yeah, I guess. But it shouldn't be boring either, right? Noone likes a boring birthday.
Some sort of special quest or minigame with special rewards? In another game there is a place called... something. Anyhow, the name is not important, after getting through you randomly get one of several great treasures from a big trophy vault (visible from the outside of the vault, the item you "win" gets teleported out). Perhaps there should be some sort of minigame in a city nearby or something where you can win a random artifact?

That would be nice, but birthday artifacts should only be slightly better than most equipment. Rolf's Savior is a good example.
