Actually puppy must be generated alive before fourth day. If you'll generate last level of PC before time limit, you can come back there later, and cute dog will be still alive.

Before fifth day. "3 days 23 hours passed" still works, so as long as you reach PC:6 on your fourth day, the puppy will be created alive. Whether it will stay that way depends on the monsters nearby and whether you can save it...
