I've never seen it cavernous either. There is a cavernous level around there, typically D:36-37 but it's the first time I see the cat level like this.

The Banshee room is an even number of tiles tall, so this occurs in [almost?] all games - the room generation algorithm can't quite handle the hookup to the room since it expects an odd number height, so you get a row of doors.
