Bug Report: Controller bindings go missing

What is the correct place to file bug reports?

Expected result: To see a full list of gamepad controls to rebind in the gamepad tab on the settings screen.
Actual result: At some point some of the controls have gone completely missing, like "Use skill" and "Previous Tab".
Steps to replicate: I don't know why it's happening. I was configuring my controls and then went in to make another change but the binding I changed was gone.
