- At least four times (Zharradan, gut, Grond)
- gut's run didn't involve any piety abuse. Zharradan's run didn't straight-up piety scum, but did sacrifice dragon-doubled gold.
- Piety scumming version doesn't need an altar. Otherwise it's technically possible without, but the chances of succeeding without one are terrible (even by brimstone man standards).
- Run away, close doors, hope they don't shrug.
Are you allowed (I guess both "ethically" and for getting the steam achievement) to step back out of the ToEF? Not to step off of the square, but to go back to the worldmap to regain PP/HP?

There is no Steam achievement for this challenge. Stepping out to heal is not allowed (or at least is considered a lesser version of the challenge).
Biggest issue would be digging. It's entirely possible for you to run out of frost bolt before managing to get a digging tool up to TF:4.
